TUI Flight Aborted Mid-Air Due to Cabin Pressure Oversight
In a concerning aviation incident, a TUI Airways flight from Manchester to Kos, Greece, carrying 193 people, was forced to turn back mid-air due to a failure in cabin pressurization. The event, which took place on October 17, 2023, highlights serious lapses in safety protocols and human error, as revealed in a detailed report by the Air Accidents Investigation Branch (AAIB).
The Boeing 737-8K5 experienced a cabin altitude warning shortly after takeoff, over northern Lincolnshire. Investigations uncovered that critical switches responsible for engine bleed air systems were left in the "off" position following maintenance the night before. This oversight went undetected during pre-flight checks by the engineers and pilots. As a result, the cabin failed to pressurize, creating a potential hypoxia risk for all onboard.

During the flight, the crew initially attempted to resolve the issue by activating the switches and continuing their climb. However, further warnings prompted the captain to stop climbing and consult ground operations. The decision was ultimately made to return to Manchester Airport. While passengers were briefed about the situation via the public address system, the AAIB noted that oxygen masks would have deployed automatically had the aircraft continued climbing to 14,000 feet. Though the risk of immediate loss of consciousness below this altitude is minimal for healthy individuals, prolonged exposure to low oxygen levels can impair cognitive performance and decision-making.
In an added layer of complexity, both pilots were on standby duty, with the captain operating on limited rest due to last-minute schedule changes. The AAIB report suggested that fatigue might have been a contributing factor, particularly given the commander’s extensive overtime duties in the preceding weeks.
This incident underscores the critical importance of strict adherence to maintenance protocols and pre-flight checks. The fact that such a significant error slipped through multiple safeguards raises serious questions about procedural compliance. The aircraft involved faced another incident three days later, veering off the runway during Storm Babet, although the two events were unrelated.
Fortunately, no injuries were reported in either case. However, these events serve as stark reminders of the high stakes in aviation safety and the need for constant vigilance at every level of operation.
